Illumina Multi-Site Confined Space Safety Assessment

Illumina, Inc. retained Citadel EHS to perform a comprehensive confined-space evaluation across four manufacturing sites in the Americas. The project was designed to identify and classify confined spaces under Cal/OSHA 8 CCR §5157 and Federal OSHA 29 CFR §1910.146, identify associated hazards, and develop recommendations for personal protective equipment and rescue methods.

Potential spaces were reviewed for physical characteristics, operating function, and present or anticipated hazards using a customized confined-space hazard-evaluation form.

For each evaluated space, Citadel determined whether it met permit-required or non-permit-required confined-space criteria and documented the classification, hazards, and relevant observations. For permit-required spaces, Citadel also developed recommendations for entrant PPE and rescue methods.

The completed report identified multiple permit-required confined spaces and recommended that Illumina update its written confined-space program to incorporate the identified spaces. Citadel also recommended coordinating implementation with existing lockout/tagout, electrical safety, and PPE programs; communicating newly identified spaces and hazards to affected employees; labeling spaces that could be inadvertently accessed; and reevaluating spaces as facility conditions or operational responsibilities change.

The assessment information was also intended to support improvements to Illumina’s confined-space entry training program. By combining standardized regulatory evaluation, facility-specific inventory development, hazard documentation, PPE and rescue recommendations, and program-level guidance, Citadel provided Illumina with an organized framework for strengthening confined-space management across multiple manufacturing operations.
